After ploppping down $35 for Fractal Mapper 8.0 and more than $100 on a suite of tools in the ProFantasy library (Campaign Cartographer 3, etc), I have to say that I have been nothing but dissatisfied with the tools I've been able to find so far. I've taken to sketching my ideas and getting my friendly neighborhood graphic designer to help me turn those into beautiful content using Adobe Creative Suite tools.
Of course, I have a distinct advantage there: my "friendly neighborhood graphic designer" happens to be my wife.

If you are looking for something for dwarven forge, you can use:

The illustrated tiles were done by me, and someone else did the tile mapping program.
